Responsibilities Coordinate the development, implementation, and ongoing evaluation of individualized care plans for our patients, ensuring that all services are patient-centered and priority-driven Collaborate with our skilled nursing staff, therapists, and other healthcare professionals to ensure seamless communication and effective treatment plans Review and maintain accurate and up-to-date records, including MDS assessments, care plans, and progress notes Participate in facility-wide quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient outcomes Provide exceptional customer service to patients, families, and healthcare providers, promoting a positive and respectful environment Stay current with regulatory requirements, industry developments, and best practices in MDS coordination, applying this knowledge to continuously improve patient care Requirements Current Registered Nurse License (RN) Strong communication, organizational, and problem-solving skills Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines Familiarity with MDS assessments and care planning processes What We Offer PTO, Holiday Pay, Health, Dental, Vision, Life, Voluntary Benefits, 401k #J-18808-Ljbffr
