Registered Nurse (RN) - Case Management

Clinical Role – Discharge Planning
Schedule: Monday–Friday
Requirements:
- RN licensure: GA or Compact license required at time of submission
- Minimum 3 years relevant experience
- BLS certification required
Roles & Responsibilities:
- Drive discharge planning and care coordination for patients.
- Conduct utilization review through concurrent and continued stay reviews.
- Monitor patient length of stay and identify barriers to discharge.
- Ensure compliance with Medicare regulatory guidelines related to patient choice and documentation requirements.
- Utilize InterQual and Milliman guidelines for decision-making in patient care management.
- Maintain awareness of regulatory compliance including CMS, HIPAA, and The Joint Commission standards.
Compliance / Notes:
- Complete core items/labs within 48 hours of contract signing.
- All compliance documents due the Tuesday prior to start.
- If compliant in time, start may be pushed 2 weeks due to bi-weekly orientation schedule.
- Holiday expectations: must be available to work major holidays as outlined in original posting.
