Registered Nurse-Women's Health Clinical Navigator

Overview The Veterans Affairs Oklahoma City Health Care System is seeking a Registered Nurse‑Women's Health Clinical Navigator for Ambulatory Care to provide and/or supervise direct patient care. Assess – plan – implement – and evaluate delivery of care based on age‑specific components. Assume responsibility for the coordination of care focused on patient education, self‑management, and customer satisfaction throughout the continuum of care. Qualifications English language proficiency; in accordance with 38 U.S.C. 7403(f) – no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed: the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE). Bridging program: Individuals attending a master's‑level bridge program in nursing who have completed coursework equivalent to a bachelor's degree may become registered as a nurse with a state licensing board prior to completion of the bridge program, and may be appointed on a temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program (see VA Handbook 5005 – Appendix G6). Foreign‑educated graduates: possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program; credit for foreign nursing education higher than associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD). Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. state, territory, Commonwealth, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements but do not possess the required licensure may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120‑day temporary appointment while actively pursuing licensure (may be extended up to two years on a case‑by‑case basis). Grandfathering Provision – all persons currently employed in VHA in 0610 series and performing the duties as described on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held. Grade determinations and scope‑education requirements are detailed for each grade and level within the online assessment ( Preferred experience: case management experience and/or women’s health experience. Physical requirements: visual acuity, keen hearing, clear speech, manual dexterity, potential for long periods of walking, standing, stooping, sitting, bending, pulling, and pushing; ability to transfer patients and objects; proficiency in use of mechanical inpatient lifts; exposure to infected patients or contaminated materials, isolation situations, operative/invasive procedures, or home settings; exposure to potentially combative patients; ability to work flexibly, muster resilience, and shift priorities based on patient needs. Must complete annual employee health requirements such as annual TB screening or testing. Responsibilities Provide patient management, care coordination, and disposition planning for Veterans, including mammography, cervical cancer screening, and/or bone density testing. Assist Veterans, family members, and caregivers in accessing appropriate services and options to meet complex healthcare needs. Coordinate care with multiple providers across all levels and sites of care, ensuring service interface within the VA and community programs for continuity of care. Provide oversight of the patient’s journey through the diagnostic and treatment continuum, ensuring scheduling, timeliness, and completion of all steps while tracking the plan of care. Coordinate and facilitate women’s health care, contact the Veteran, facilitate appointments and authorizations, track receipt of test results and other medical documentation, and notify providers of needed follow‑up care. Interface with outside referral agencies and ensure their business and clinical practices follow regulatory standards. Anticipate and implement preventive patient care measures. Provide education, guidance, and support to Veterans and caregivers while collaborating with interdisciplinary professionals across all levels of care. Address psychosocial, nursing, and medical needs of Veterans and their families/caregivers through participation within the interdisciplinary care team. Notify the Nurse Manager of any safety incidents, employee injuries, or concerns of team members not providing care according to the appropriate Standards of Care. Perform additional duties as assigned. Benefits Competitive salary with regular salary increases and potential for performance awards. 50 days of paid time off per year (26 days annual leave, 13 days sick leave, and 11 paid Federal holidays). Traditional federal pension (5‑year vesting) and federal 401(k) with up to 5% VA contribution. Federal health, vision, dental, term life, and long‑term care insurance. Full, unrestricted state license required. Work schedule: Monday through Friday, 0730‑1600. Telework: not available. Virtual: not a virtual position.
