Care Management Assistant II - BSW - Multiple Positions Open!

About the Role The Care Management Assistant II supports Case Managers and Social Workers by ensuring the accuracy of patient and insurance information, facilitating reimbursement, and expediting patient stays. The assistant also maintains post-acute network relationships across the Cone Health enterprise and helps improve transition of care. Responsibilities Provide collaborative administrative support, filling out appropriate documentation and contacting patients to reduce clinical staff workload. Prepare referral forms and other documents, ensuring proper grammar and spelling. Correspond with patients, caregivers, insurance companies, post-acute vendors, and community agencies under the guidance of a Case Manager or Clinical Social Worker. Ensure accuracy of submitted information and notify departmental leaders of potential coverage gaps. Develop strong business relationships with post-acute network partners (SNF, LTAC, IRF, etc.) and contribute to business development with VBCI leadership and Cone Acute facilities. Collaborate across acute care facilities, post-acute resources, and community partners to deliver an exceptional patient admission experience. Perform other duties as assigned. Qualifications Education: Required – LPN training program or Bachelor of Social Work. Experience: Preferred – 1 to 2 years in care management or post-acute support. Licensure / Certification Current LPN licensure required as per state regulations. BSW degree or BLS certification acceptable. Locations & Shifts Locations: Alamance Regional, Moses Cone, Wesley Long Hospital. Full‑time 12‑hour shifts (Day or Night) with weekend rotation (36 hours/week).
