Home Care Marketer

Your Choice At Home is seeking a motivated and outgoing Home Care Marketer to help grow our home care services by building strong relationships with healthcare professionals, referral sources, community organizations, patients, and families.

This position focuses on marketing, business development, community outreach, relationship building, and generating new patient referrals. The ideal candidate is someone who enjoys networking, making connections, visiting referral partners, and representing an organization throughout the community.

Productivity Bonus

Eligible for a performance-based bonus tied to productivity and successful patient growth.

Key Responsibilities

• Develop and maintain strong relationships with hospitals, rehabilitation facilities, physicians' offices, social workers, discharge planners, assisted living communities, senior centers, and other referral sources.
• Identify and develop new referral opportunities to increase patient census.
• Conduct regular phone, email, and in-person outreach with referral partners.
• Visit healthcare facilities, physician offices, community organizations, and other potential referral sources to promote Your Choice At Home services.
• Educate referral sources, patients, and families about available home care services.
• Follow up on leads and referrals and help connect prospective patients with the appropriate team.
• Represent Your Choice At Home at community events, healthcare events, senior expos, networking events, and other outreach opportunities.
• Maintain strong relationships with existing referral partners and identify opportunities for continued growth.
• Track referral activity, outreach efforts, and relationships with referral sources.
• Assist with marketing materials, social media, community promotions, and other outreach initiatives as needed.
• Collaborate with leadership and internal teams to support census growth and ensure a positive experience for referral partners, patients, and families.
• Stay informed about community resources, healthcare organizations, and local opportunities that may generate new referrals.

Qualifications

• Experience in healthcare marketing, home care, home health, sales, business development, account management, or community outreach required.
• Strong communication, networking, and relationship-building skills.
• Comfortable making phone calls and conducting frequent in-person visits.
• Self-motivated, organized, and able to manage multiple relationships and priorities.
• Professional, personable, outgoing, and comfortable representing an organization in the community.
• Strong follow-up skills and a results-oriented mindset.
• Healthcare or home care experience is a plus.
• Bilingual or multilingual candidates are encouraged to apply.
• Valid driver's license and reliable transportation may be required.
