You will play a key role in helping patients regain independence and achieve personalized therapy goals.###### **Key Responsibilities*** Perform **patient evaluations** and develop individualized treatment plans* Deliver therapeutic interventions to improve strength, balance, and mobility* Monitor patient progress and adjust treatment plans as needed* Maintain accurate and timely documentation* Collaborate with interdisciplinary teams to support patient outcomes* Educate residents and families on therapy techniques and recovery goals###### **Qualifications*** **Bachelor's degree required**; advanced degree preferred* Active, unencumbered **Physical Therapist license in South Carolina*** Experience in a **long-term care or rehab setting preferred*** Familiarity with **PointClickCare (PCC) and Casamba** preferred* Strong communication and clinical skills* Ability to work independently and as part of a team###Join a team where your skills are valued and your work truly makes a difference.
