You will collaborate with physicians, families, and interdisciplinary team members to deliver high-quality, patient-centered care.### **Key Responsibilities*** Perform **patient evaluations** and develop individualized treatment plans* Provide therapy to improve **strength, balance, mobility, and function*** Monitor patient progress and adjust treatment plans as needed* Maintain accurate and compliant **clinical documentation*** Communicate effectively with physicians, residents, families, and care team members* Participate in **care planning and discharge planning*** Provide education and guidance to residents and staff### **Qualifications*** Degree in **Physical Therapy** (Bachelor’s required; Master’s or Doctorate preferred)* Active, unencumbered **Physical Therapist license in South Carolina (required)*** Must maintain all continuing education and remain in good standing with the State Board* Experience in a **skilled nursing or rehab setting preferred*** Familiarity with **Net Health and PointClickCare (PCC) preferred*** Strong clinical, communication, and organizational skills* **CPR certification** (if required by licensure)### ### ### **Why Join Greenville Post Acute?**At GPA, you’ll enjoy the **flexibility of PRN work** while being part of a team that truly **cares about residents and each other**.
