Case Manager, Housing First and Recovery Support

Montgomery County Coalition for the Homeless (MCCH) seeks a dedicated Case Manager to support formerly homeless individuals living in Hope Housing programs. You will maintain an active caseload, coordinate services, and apply a housing-first, trauma-informed approach to help clients achieve stable housing and life skills. Responsibilities include implementing 30-day service plans, conducting assessments, liaising with service providers, managing HMIS data, and collaborating with landlords and #J-18808-Ljbffr Montgomery County Coalition for the Homeless
