About the Role:

As a Nurse Practitioner (NP) or Physician Assistant (PA) with Carease Health - OH, you will work in a hybrid setting that combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Your primary responsibilities will include delivering preventative care to residents in skilled nursing facilities, building strong relationships with facility staff, and maintaining a patient-centered approach.

Key Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities

• Build strong relationships with facility staff and maintain a patient-centered approach

• Collaborate with a dedicated scribe team for efficient documentation

• Monitor patient progress and adapt care plans as needed

• Communicate effectively with patients, families, and healthcare teams

Requirements:

• Active NP or PA license (in the state of practice)

• 1+ year of experience in Geriatric care and Documentation review

• Excellent communication, collaboration, and teamwork skills
