LVN / LPN - LTC/SNF

Job Summary:

The LVN provides fundamental nursing care within a skilled nursing facility, working under the supervision of the Director of Nursing. This role involves collaborating with the healthcare team to deliver personalized patient care and ensuring the safety and well-being of residents.

Responsibilities:

• 
  Conduct comprehensive patient assessments and prioritize data collection based on immediate needs.
  
  
• 
  Develop and implement individualized care plans in collaboration with the healthcare team.
  
  
• 
  Perform clinical procedures such as blood draws, EKGs, IV starts, and medication administration accurately and timely.
  
  
• 
  Monitor patient conditions, document assessments, and respond promptly to abnormal findings or emergencies.
  
  
• 
  Provide patient and family education to promote understanding and participation in care.
  
  
• 
  Maintain safety, infection control, and confidentiality standards throughout all care activities.
  
  

Qualifications:

• 
  Licensed as an LVN or LPN in California with current BLS CPR certification.
  
  
• 
  Demonstrate good physical health, stamina, and manual dexterity required for patient care.
  
  
• 
  Possess strong assessment, communication, and problem-solving skills.
  
  
• 
  Able to tolerate behavioral challenges from patients and adapt to changing healthcare needs.
  
  
•
