SUMMARY

The Los Angeles County Department of Homeless Services and Housing (HSH)consolidates our countywide response to homelessness. The driving force behind HSH is increasing accountability and transparency, improving care for people experiencing or at risk of homelessness, and streamlining collaboration with partners including services providers, the County’s 88 cities, and unincorporated areas to deliver high-quality, life-saving care. Staff schedules are based on business need and may include the option of a hybrid work schedule where employees work remotely and from the office.

The Homeless Prevention Unit (HPU) is a collaborative effort between HSH, California Policy Lab (CPL) and the Department of Mental Health (DMH). The HPU proactively identifies individuals who are most at risk of homelessness through predictive analytics and provides them with flexible, early interventions to resolve issues that might lead to loss of their housing. HPU is expanding to serve young adults between the ages of 18-24 in need of proactive homelessness prevention resources. The HPU has significant interactions with several County departments to address challenges navigating service systems and/or to connect or re-connect individuals to services to ensure housing stability; therefore, the ideal candidate will be knowledgeable in mainstream and specific programs and services tailored to young people at risk of or experiencing homelessness while being flexible to meet the changing needs and demands of the program and this population.

The Assistant Staff Analyst (Transition Age Youth (TAY) Case Management Program Manager) will serve as a supervisory program manager for the HPU TAY program and will report to the Associate Director. Working closely with HPU program management and HSH management, this position will be responsible for developing and implementing program structures, processes and evaluation measures and will assist in managing day-to-day operations for the Unit. The TAY Case Management Program Manager will supervise a team of TAY Case Managers who interface directly with HPU TAY clients and will be responsible for designing and implementing policies and procedures to assist clients with complex mental health needs, legal barriers, income barriers and/or housing insecurity. This role will require building a new team who is tasked with serving young people in Los Angeles County through a client centered model and developing policies and guidance that will help shape the TAY program of the HPU.

The ideal candidate has experience working with TAY who are/have experienced homelessness or at risk of homelessness. And has a deep understanding of evidence based best practices such as Trauma Informed Care and Harm Reduction.

ESSENTIAL FUNCTIONS

Case Manager Supervision

• Oversees a team of HPU Case Managers and supports their efforts through regular supervision check-ins to determine and implement interventions and linkages to housing and services for HPU TAY clients.
• Supports HPU Case Managers and their clients to identify social, emotional, and economic factors which may interfere with obtaining maximum benefits from medical and mental health care.
• Reviews and monitors client outcomes with Case Mangers to address gaps in program delivery, documentation, and data input.
• Supports individual Case Managers in identifying their strengths, areas of growth and helps facilitate professional development.
• Supports in the development of tailored pathways to vital resources across LA County such as mental health care, legal services, housing assistance, educational resources, etc. That will help stabilize housing for TAY clients.

Leadership and Program Support

• Assist HPU program management with developing and implementing program processes for HSH, including day-to-day operations, trainings, policy and procedure development, reporting requirements and quality improvement/evaluation components.
• Builds policies and processes for HPU team members to follow to support clients with complex health and mental health needs.
• Assists HPU program management with developing the operational structure and growth of the HPU.
• Assist HPU program management with evaluating program efficacy and progress, and help develop, recommend and implement program modifications.
• Assist HPU program management with coordinating and facilitating regular meetings and communications for HPU and program partners to ensure efficient and thorough processes to promote integrated, seamless and coordinated systems of care.
• Promotes a safe and inclusive work environment for the entire HPU Team by collaborating with HPU management and Team Leaders to foster culture of care.
• Works collaboratively with other Housing Navigator and Community Engagement and Partnerships Managers to ensure coordination of care between Case Managers and HPU Management.
• Works collaboratively with Associate Director and Lead Program Managers to implement projects that impact TAY programing.
• Other duties as assigned.

JOB QUALIFICATIONS

One year of experience as a Medical Case Worker I - OR - A Bachelor's degree from an accredited college or university and one year of professional casework experience interviewing, counseling, and assisting patients/clients with social problems.

Certificates/Licenses/Clearances

• A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
• Successful clearance of the Live Scan process with the County of Los Angeles.

Other Skills, Knowledge, and Abilities

• Strong data analysis, problem solving, various data systems and how they interact, deep knowledge of the landscape of mainstream and safety net services utilized by TAY.
• Strong relationship management skills with a desire to work in a fast-paced mission driven environment.
• Knowledgeable of community, mental health, and mainstream systems and the variety of programs offered by each to assist those most at risk of experiencing homelessness.
• Highly motivated self-starter, with proven ability to develop creative solutions.
• Ability to coordinate projects and competing priorities in a high-pressure, fast paced and time sensitive environment.
• Ability to interact and work closely with a diverse clientele.
• Demonstrated experience interviewing clients, individuals, and families to collect data to determine what services are needed.
• Demonstrated experience in linking clients, individuals, and families to specific healthcare, mental health and community-based programs.
• Ability to pay close attention to detail.
• Ability to work in a professional manner, with courtesy and tact, and maintain confidentiality and security of documents and information.
• Ability to work collaboratively with multidisciplinary professionals and partners both internally and externally.
• Must possess strong organizational skills initiative and follow-through.
• Computer literate and skilled in use of MS Windows-based programs, including MS Office (Word, Excel, PowerPoint).
• Knowledgeable of data systems utilized across the homeless system of care.

• The HPU operates in a hybrid work environment. Team members are expected to work from the HSH office, Downtown Los Angeles, a minimum of twice per pay period, with additional in-person meetings, events, and client field-based meetings as needed.
