Who You Are:

We are seeking a candidate with a strong positive attitude toward customer service to act as a Social Worker for Home Health. The right candidate will always strive to do the best job possible, to be creative, and to work towards accomplishing facility goals.

Position Summary :

The Medical Social Worker provides medical social services to the patient and/or family according to the Plan of Care. Performs all assignments with honesty, integrity, and care that promotes the mission, vision, and values of Friendship Village.

Education and Experience:

At a minimum, a home health medical social worker must hold a Licensed Master Social Worker (LMSW) credential issued by the Missouri State Committee for Social Workers. This requires a Master of Social Work (MSW) degree from a program accredited by the Council on Social Work Education (CSWE) and a passing score on the Association of Social Work Boards (ASWB) Master’s-level exam. Federal Conditions of Participation (42 CFR § 484.115(m)) also require at least one year of social work experience in a healthcare setting.

• Home Health or Hospice experience preferred.

• Fosters an environment honoring resident/patient preferences and choices.

• Exhibits well-developed communication skills.  Can professionally receive and follow oral instructions in English. Able to understand and use simple math.

• Ability to cope with mental and emotional stress related to the position, function independently, and have flexibility and professionalism.

• Demonstrates the ability to develop and maintain good working relationships.
