Role: MDS Coordinator RN Responsibilities: Lead assessment and care planning for residents in senior living and rehabilitation settings. Complete and manage MDS assessments, ensuring accuracy and compliance with regulations. Collaborate with interdisciplinary teams to develop personalized care plans. Support quality improvement initiatives and promote reimbursement accuracy. Mentor nursing staff on documentation standards. Qualifications & Skills: Nursing licensure (RN), knowledge of MDS processes, regulatory compliance, and documentation standards. Strong collaboration, attention to detail, and commitment to patient-centered care. Key Duties: Conduct assessments, ensure regulatory adherence, develop individualized care plans, support quality initiatives, and mentor staff in documentation practices. #J-18808-Ljbffr
