LPN Crisis Care (Nights) Full Time Direct Patient Care Columbus, OH, US LPN – Crisis Care Location: Columbus, Ohio Schedule: Full-Time | Nights | 8:00 PM–8:00 AM Shifts: Three 12-hour shifts per week | Every-other-weekend rotation Make a Meaningful Difference at the End of Life At Ohio’s Hospice of Central Ohio, we believe every patient deserves compassionate, exceptional care throughout their end-of-life journey. We are seeking an LPN – Crisis Care who has a genuine heart for hospice and is committed to providing comfort, dignity, and support when patients and families need it most. As a Crisis Care LPN, you will provide one-on-one bedside care for patients who are actively declining and require an increased level of support. What You’ll Do Working under the direction and supervision of an RN, the Crisis Care LPN will: Provide high-quality nursing care to hospice patients in homes and facilities. Provide one-on-one bedside care for patients experiencing an active decline. Monitor patient condition and communicate changes to the supervising RN and care team. Provide comfort-focused care that supports the patient's physical, emotional, and end-of-life needs. Work collaboratively with patients, families, caregivers, and the interdisciplinary hospice team. Document patient care and services accurately and timely. Support patients and families with compassion, dignity, and respect throughout the end-of-life journey. Deliver care in accordance with hospice standards, policies, and patient-centered goals. What We’re Looking For Licensed Practical Nurse (LPN) license in the State of Ohio with no active board actions. One year of nursing experience preferred. Current CPR certification. Computer skills sufficient to accurately document patient care and services. Ability to drive safely during daytime, nighttime, and inclement weather conditions. Valid driver's license with a safe driving record. Minimum state-required automobile insurance coverage. Ability to successfully pass a background check.
