Housing Outreach & Rapid Rehousing Navigator
Care-Resource-Community-Health-Centers,-Inc.
Care Resource Community Health Centers, Inc. seeks a dedicated liaison/coordinator to support clients facing homelessness by coordinating housing and medical services.
