Street Medicine Outreach Case Manager - Miami Impact

Camillus Health Concern is seeking a compassionate Case Manager for the Lazarus Street Medicine Outreach Project in Miami. You will provide day-to-day street-based outreach and psychosocial assessment to connect individuals experiencing homelessness with healthcare, housing resources, and social services.
