People Incorporated Mental Health Services is hiring a Case Manager II - Waiver Case Management (Anoka County) in Minnesota. This full-time role (Hybrid in Minneapolis area) supports clients with chronic mental illness by connecting them to community resources and helping them live independently.

You will conduct assessments, review plans, and coordinate ongoing services, documenting all actions. Qualifications include a Bachelor’s degree in a related field and at least 1 year of relevant
