Case Manager II - Medicare & Waivers for Housing Stability

Agate Housing

Agate Housing in Minneapolis is hiring a Case Manager II - Medicare & Waivers to support MPHA residents across the lifespan with health coverage, waivers, and long-term services. You will provide resident-centered case management, assist with applications and renewals, coordinate with county staff, plans, and providers to secure benefits and services.

This is a full-time, in-office role with occasional remote work, $26/hour, AFSCME Local 999 represented, and benefits including medical, vision,
