Responsibilities
- Conduct comprehensive occupational therapy evaluations for residents and formulate individualized treatment plans.
- Implement interventions to improve activities of daily living, fine motor skills, cognition, and functional mobility.
- Monitor progress, adjust treatment plans as needed, and document outcomes in resident records.
- Collaborate with an interdisciplinary team (nursing, physical therapy, speech-language pathology, social work) to coordinate care and discharge planning.
- Educate residents and families on home exercise programs, safety strategies, and activity modifications to promote independence.

Requirements
- Active Occupational Therapist license with ability to practice in the assignment state.
- Degree in Occupational Therapy from an accredited program.
- General OT experience; LTC or geriatric population experience preferred.
- Strong assessment, documentation, and communication skills.
- Ability to work effectively in a fast-paced, team-based long-term care environment.
