Home-Based Primary Care NP for Geriatrics

You will work within a team-driven, physician-led model and focus on patient-centered care rather than volume. The role requires state licensure as an NP, DEA license eligibility, CPR/BLS, and a valid driver’s license with a clean record. Geriatrics experience is preferred.
