Responsibilities

• Assess patients’ healthcare needs and develop a comprehensive, interdisciplinary care plan with the patient and family.
• Facilitate communication among patients, families, and healthcare providers; coordinate transitions between care settings and plan for post-hospital care.
• Evaluate medical necessity for treatments and hospital stays; collaborate with insurers to obtain authorizations and identify discharge barriers.
• Advocate for patients’ needs and preferences; educate patients and families about diagnoses, treatments, and self-care; connect with community resources.
• Maintain thorough documentation of assessments, interventions, and outcomes; participate in interdisciplinary care conferences and coordinate plan implementation across disciplines.

Requirements

• Active Oregon RN license
• CRASE training
• Education: Graduate of an accredited nursing program or related medical occupation (BSN/MSN preferred)
• Experience: Previous clinical experience in an acute care or home health setting; discharge planning/case management experience preferred
• Language: Bilingual (English/Spanish) preferred
• Ability to work in an inpatient setting and collaborate with a wide healthcare team
