Dialysis RN (Transitional Care) - Home Therapies

Fresenius Medical Care Grayslake Clinic in Illinois seeks a Registered Nurse to support the Transitional Care Unit (TCU) for home dialysis patients. You will assess, educate, and coordinate care to ease the switch to dialysis and improve outcomes, working with the End Stage Kidney Disease team. This role offers entry-level CAP progression with RN licensure, CPR/BLS, and home dialysis experience preferred.
