Nurse Practitioner – Advanced Wound Care (Full-Time | Home-Based Care)

Based in Lake Villa, IL | Serving Lake, McHenry, Cook, DuPage, DeKalb, and Winnebago Counties

Salary: $125,000/year

4 or 5-day work week | 25–30 patients/week

Wound Wellness PLLC is a nurse practitioner-owned and operated wound care practice founded in February 2024, providing advanced wound care in the comfort of patients' homes and in-clinic . We are a mission-driven, rapidly growing startup seeking a full-time Nurse Practitioner who is compassionate, motivated, and ready to make a difference.

Position Highlights:

• Provide advanced wound care including:
• Sharp debridement
• Ultramist therapy
• Wound vac application
• Biopsies
• Tissue graft placement
• See 25–30 patients per week
• Flexible schedule – choose 4 or 5 working days
• Maintain weekly continuity of care
• Visits primarily conducted in patients’ residences
• Looking for someone to service the Chicago area

Ideal Candidate:

• Licensed Nurse Practitioner in the state of Illinois
• Live in or nearby the Chicago area
• New grads welcome!
• Home health or mobile wound care experience preferred
• Strong work ethic and adaptability
• Comfortable working independently
• Passionate about patient advocacy and continuity of care
• Excited to contribute to a growing and evolving practice
