Position Summary

Join a highly engaged, mission-driven team providing comprehensive care in a dedicated Clinic or Hospital setting. You will work closely with committed physicians who prioritize mentorship, professional development, and team-based learning. This role offers an opportunity to collaborate with staff members to support seamless patient care. You will make a meaningful impact serving as a trusted partner to patients and families navigating health care needs.

Coordinates and delivers primary care in collaboration with the Practice Site Physician by:

• Working within their approved scope of practice.
• Obtaining complete medical history and physical data on patients.
• Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures as needed.
• Performing a complete physical exam and recording findings. Collaborating with physicians in managing the medical needs of patients.
• Ordering appropriate laboratory and diagnostic procedures.
• Educating and advising patients regarding health and illness prevention. Recommending community resources to meet patient and family needs.
• Support patient access and care coordination through triage, follow-up, and management of urgent situations

Provides clinical and administrative assistance to BMG programs, e.g., education, follow-up, and the like by:

• Participating in health education.
• Maintaining related records and statistical data and preparing statistical reports as needed.
• Participating in interdisciplinary care conferences and assisting with referrals and dissemination of information to appropriate vendors.
• Assisting in the development and coordination of ongoing education and follow-up programs designed to measure client outcomes.

Required Credential(s):

• Graduate of an accredited Physician Assistant or Nurse Practitioner program.
• Current, valid state licensure and appropriate certification (NCCPA or ANCC/AANP) obtained prior to hire date or job transfer date. Verified by local Credentialing Office.
• BLS obtained within 1 Month (30 days) of hire date or job transfer date if required. American Heart Association or American Red Cross accepted.
• Current BLS and ACLS certifications if required.
• Ability to work collaboratively in a fast-paced, team-oriented environment.
