Job Requirements

• Minimum 2 years of recent hospital case management and discharge planning experience
• Active California Social Worker license
• Current BLS certification through the American Heart Association
• Strong knowledge of care coordination , utilization review, discharge planning, and patient advocacy
• Ability to collaborate effectively with interdisciplinary teams and external review agencies
• Excellent communication, assessment, and organizational skills

Responsibilities

• Coordinate patient care management to ensure optimal utilization of resources and timely service delivery
• Perform comprehensive patient assessments and develop individualized care plans
• Support care plan implementation, ongoing evaluation, and continuity of care
• Facilitate discharge planning and coordinate appropriate post-acute services and resources
• Work closely with interdisciplinary teams to promote quality outcomes and patient satisfaction
• Ensure compliance with external review agencies and organizational standards
• Advocate for patients by addressing clinical, psychosocial, and resource-related needs
• Promote cost-effective care while honoring patient preferences and holistic needs
