Social Services Coordinator

You have an exciting opportunity to join our team and work at our exceptional Skilled Nursing Facility! We are currently expanding our staff and are looking for a compassionate and enthusiastic Social Services Assistant! Responsibilities Essential Functions: Develop a comprehensive social history assessment, a psycho-social assessment, discharge plan, and complete the MDS and Care Plan for new admits. Orient and assist new residents in adjusting to the facility. Complete MDS forms in a timely fashion Assist residents and families with adjustment to changes, including room changes. Assist families with the engraving and/or marking of residents' personal items. Review residents' rights with residents and staff and attend to various personal needs of the residents. Assist in the development of a volunteer program and assist in facilitating family group meetings. Document progress notes that relate to each resident's care plan when necessary and within policy timeframe Comply with all Company policies related to safety and infection control procedures including the proper use of mechanical lifts, gait belts and personal protective back supports Understand, comply with and promote all rules regarding resident rights, promote positive relationships with residents, visitors and regulators, to include presenting a professional appearance Other duties as assigned or needed Skills Proven work experience in long term care Working knowledge of social theories and practices Social perceptiveness and empathy Ability to build and maintain professional helping relationships Ability to relate and communicate with diverse population and groups Resilience along with ability to assess situations Willing to submit to background checks Psychology or sociology experience preferred Job Type: Full-time Benefits: 401(k) Dental insurance Health insurance Paid time off Referral program Vision insurance Medical Specialty: Geriatrics Schedule: 8 hour shift Experience: Long Term Care: 2 year #J-18808-Ljbffr Emerald Nursing & Rehab Brookside
