As a Licensed Practical Nurse, you will play a crucial role in ensuring the health and well-being of patients by performing and documenting assessments, completing health history and admission processes, and identifying discharge planning and education needs. You will work closely with a multidisciplinary team to provide integrated, patient-centered care.

• Assist registered nurses or other licensed practitioners in identifying patient needs and care priorities
• Contribute to the evaluation and modification of care plans
• Demonstrate competency in department-specific functions
• Assign or delegate nursing interventions to qualified personnel
• Identify early warning signs of changes in patient conditions and report appropriately
• Implement nursing interventions and prescribed medical regimens safely and timely
• Collaborate and communicate with patients, families, and team members to ensure quality care
• Participate in health education and discharge planning
