What You’ll Do

Perform occupational therapy evaluations and develop individualized plans of care

Help patients improve their ability to safely perform activities of daily living and regain functional independence

Assess home safety, recommend adaptive equipment, and provide caregiver education

Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team

Complete accurate and timely documentation while maintaining Medicare and agency standards

Qualifications

Current Florida Occupational Therapist (OT) license

Valid driver’s license and reliable transportation

Excellent communication and organizational skills

Home health experience preferred, but not required

Experience with WellSky or another home health EMR is a plus
