FTC Facilitator – Saint Dominic's Family Services FTC FACILITATOR – Full Time / Part Time, Regular Full Time, Total Hours Per Week: 35, Salary: From $55,000 To $55,000. Duties & Responsibilities Facilitate and lead the process for engaging families, community members, and service providers in critical decisions related to safety and risk. Accurately track FTC information, reschedules, and cancellations. Contribute knowledge about the needs, strengths, and resources which may be present to ensure the child(ren)’s safety while supporting the family and giving them access to services in the community. Contact and consult with ACS, agency personnel, and service providers as needed to schedule ACS related (Trial Discharge, Final Discharge, Parole, Placement Change) FTCs. Lead conference participants in the group process and facilitate discussion to ensure the best decision regarding the safety and well-being of the child by guiding permanency discussions and developing a plan with the family. Effectively manage conflict and strong emotions as they arise by utilizing conflict management skills while directing the group process and structure of the conference. Accurately record FTC information and decisions on designated forms (FTC Summary Report) and complete the Service Plan Review tab in Connections within 48 hours of each conference. Help the family understand the agency’s concerns in relation to safety and risk. Promote an inclusive, safe, and supportive environment where all participants are assured they can be heard. Work with the accountability department to ensure documentation of all FTCs is completed in a timely manner. Debrief with case planning staff following each conference to ensure conference goals are met. Education & Experience Bachelor’s degree in social work or a social service field with at least 2 years of experience working in child welfare.
