Community Case Manager: Partnerships & Client Recovery
The Salvation Army
The Salvation Army Stapleton Corps seeks a Case Manager to partner with social service agencies, churches and nonprofits across Staten Island to develop individualized plans of recovery for families served by the Stapleton Corps Social Ministries. You will manage a caseload, perform intake and assessments, coordinate services, maintain confidential records, and report progress to supervisors while collaborating with staff to measure impact and improve outcomes. #J-18808-Ljbffr The Salvation Army
