Join #TeamAcenda and make a lasting impact.**Job Overview**Provides individual counseling and support to consumers through symptom assessment, wellness planning, and the use of evidence based practices to promote recovery and stability. Collaborates closely with the PACT team to support medication management, monitor side effects, and deliver coordinated, long term care while participating in daily team meetings. Responds to consumer needs with strong communication, teamwork, and availability for crisis assessment and intervention as part of a shared on call rotation.As an integrated health organization Acenda values the use of evidence-based practice or EBP's.**Program Info**Program for Assertive Community Treatment (PACT) provides comprehensive treatment to people with serious and persistent mental illness. People served by PACT receive individualized multidisciplinary services that include staffing available 24 hours a day/ 7 days a week, and a staff Psychiatrist.**Key Responsibilities:*** Provide individual sessions to consumers through symptom assessment, management, and supportive counseling, as well as assistance to the consumer regarding Wellness Recovery Action Plans (WRAP), and/or other crisis planning tools.* Support medication delivery and collaboration with PACT team to provide long term support to the consumer.
