Job Description & Requirements Specialty: Master of Social Work Discipline: Social Work Start Date: 06/22/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Requirements Minimum 1+ year of Acute Care Case Management/Social Work experience required MSW degree required Active Washington LMSW/LCSW license required EPIC charting experience required Experience with discharge planning and care coordination required Must be willing to float between units/campuses Every other weekend required Preferred Experience Med Surg, ED, ICU, NICU, or Mother-Baby case management experience Experience with biopsychosocial assessments Experience coordinating post-discharge services and DME Strong care coordination and documentation skills Certification Requirements MSW – Required WA Licensure (LASW, LICSW, LSWAIC, LSWAA accepted as applicable) BLS preferred Job Duties Conduct biopsychosocial assessments and initial evaluations Coordinate discharge planning and patient care transitions Collaborate with multidisciplinary healthcare teams Arrange home care services, rehab placement, and community resources Maintain accurate documentation in EPIC Support patients with insurance, transportation, and care coordination needs Benefits Weekly pay Medical benefits #J-18808-Ljbffr LanceSoft, Inc.
