Legacy Hospice is seeking a compassionate Registered Nurse Case Manager to provide high-quality end-of-life care to patients in their homes. The RNCM will lead comprehensive assessments, develop individualized care plans, and coordinate interdisciplinary care with physicians, nurses, social workers, and other team members. The role requires active RN licensure in the state, preferably BSN or CHPN, and at least 1 year of hospice/home health experience. #J-18808-Ljbffr Legacy Hospice
