Outpatient RN Case Manager — Care Coordination Lead

Harbor Healthcare System in Overland Park, KS seeks a qualified RN Case Manager for an outpatient position. You will work with an interdisciplinary team to admit, assess, coordinate care, and manage discharges and pronouncements for patients and families. Qualified candidates are a Kansas-licensed RN with 1+ year experience (hospice or home health preferred), strong communication, and reliable transportation for travel.
