Hospice Nurse Case Manager: End-of-Life Care Coordinator

Prairie Wind Hospice is seeking a Nurse Case Manager to coordinate and provide exceptional hospice care to patients and families. The role works with the Director of Nursing and the Interdisciplinary Team to deliver hands-on nursing, assess pain and symptom management, and support dignified end-of-life care. You will educate families, participate in interdisciplinary meetings, update the Plan of Care, and coordinate services to meet patient goals while upholding the hospice philosophy. #J-18808-Ljbffr Prairie Wind Hospice
