Care Coordination Services Consultant

This position provides intensive and professional case management services to youth utilizing a strength‑based, comprehensive model. The role involves linking children and families to resources, monitoring services, actively advocating for children and families, and assisting them in navigating multiple systems of care. Major Responsibilities and Duties Provide safe and effective services and adhere to safety and boundary policies. Attend safety and abuse risk management training as assigned. Follow procedures for managing high‑risk activities and supervising service recipients. Report suspicious or inappropriate behaviors and policy violations; comply with mandated abuse reporting requirements. Provide case management services for Care Coordination. Develop and maintain effective working relationships with families, community resources, medical, mental health, substance abuse professionals, child welfare personnel, juvenile justice staff, lawyers, therapists, law enforcement, school officials, neighbors, and religious and youth organization representatives. Conduct assessments to determine program eligibility, problem areas, and areas of strength. Maintain current knowledge of formal services, resources, and supports in the local community. Identify needed services and link youth to those services; identify informal supports and assist youth and family members in accessing them. Monitor linked services to ensure quality of care. Act as a youth and family advocate, empowering them to advocate for themselves. Assist youth and family members in navigating various systems and educate them to build understanding; support permanency planning and outcomes. Provide support for developing functional skills for youth and family members. Develop, monitor, and review service plans and create subsequent plans as needed to facilitate progress. Maintain appropriate direct contact with youth and family members per program requirements; remain available for crisis intervention 24/7. Create and manage discharge planning, focusing on safety, permanency, and well‑being, including step‑down placements, reunification, adoption, or transition plans and coordination of community resources. Assist in building on spiritual and religious beliefs or practices as sources of strength. Promote professionalism through role modeling and respect. Meet with youth and family members at various locations; use own vehicle to travel to work destinations and transport youth as authorized. Prepare and submit reports concerning case load status, budget requests, changes in service plans, and any unusual incidents; report critical incidents involving youth, family members, or staff per standard guidelines. Prepare proper documentation including case notes, assessments, service plans, outcome measures, narratives, and reports in a timely manner. Ensure compliance with all contractual, regulatory, program, and accrediting body standards. Notifies supervisor of changes in professional status or standing that may affect job status or program compliance. May be trained to become a certified Common Sense Parenting Instructor; as such, may conduct CSP courses at assigned locations. Knowledge, Skills, and Abilities Knowledge of regulatory requirements governing child welfare processes and services. Knowledge of community resources and service providers available in the area. Strong communication skills with individuals at all levels; professional manner while maintaining confidentiality. Ability to make quick decisions in sensitive and critical areas while maintaining procedural and regulatory integrity. Knowledge of the Boys Town Model. Computer skills in Microsoft Office. Required Qualifications Bachelor’s degree in Human Services or a related field. Minimum 1 year of experience working with children and families. Valid driver’s license with a good driving record. Own transportation to meet travel requirements; meet auto insurance requirements established by Boys Town policy and/or State and local laws and pass an annual Motor Vehicle Registration (MVR) check. Availability to provide crisis response and to work non‑traditional hours including early mornings, nights, weekends, and holidays. Florida: Must be a Certified Behavioral Health Case Manager (CBHCM) or obtain certification within 6 months of hire. Louisiana: CPR certification obtained during pre‑service training per contract requirements. Additional education or experience may be required due to state or contractual requirements. Preferred Qualifications Experience working with children and families in community‑based programs or Boys Town programs. Bilingual language skills. Other relevant experience or education.
