This role requires collaboration within an inpatient physical rehab or LTAC facility. The chosen candidate will be responsible for ensuring patient progression through the continuum of care, from admission through discharge, and will effectively manage resources in an acute hospital setting.

Key Responsibilities of the RN - Case Management

• Collaboration with patients, families, and multidisciplinary teams to ensure patient progression.
• Development of individualized care plans from admission through discharge.
• Management of patient resources efficiently and cost-effectively.
• Conduct interdisciplinary rounding and family meetings.
• Engage in utilization reviews and handle insurance authorizations.
• Coordinate discharge planning and make appropriate referrals for DME, HH, or outpatient services.

Requirements of the RN - Case Management

• Registered Nurse License or Social Worker License (LSW/LCSW) in Illinois
• Basic Life Support (BLS) certification
• A minimum of 2 years of experience in a healthcare setting, preferably in acute care
• Experience in discharge planning and care coordination
• Proficiency in EPIC charting system
