Description
Otter Tail County is accepting applications for a full-time Long-Term Care Nurse. Otter Tail County Public Health provides population-based public health nursing services that promote, protect, and improve the health of individuals, families, and communities. This position assesses community health needs, emphasizes prevention and social determinants of health, collaborates with community partners, and supports public health initiatives. This includes advocacy, policy development, and health planning activities in alignment with public health standards and community health priorities. This position is part of the Long-Term Care Unit and is assigned to the Interdisciplinary Public Health/Human Services Unit and includes, but is not limited to, providing care coordination to persons who are enrolled in managed care health plans. Care coordination includes assessment, care plan development, arranging and/or coordinating supports and services which may include preventative, routine, and/or long-term care health and service needs.

This is a full-time, 40 hours/week position, and generally the work hours are Monday-Friday 8:00-4:30 p.m. Below is the 2026 starting salary depending on qualifications: Registered Nurse with Bachelor's Degree and qualified experience: $36.94 with potential career advancement to $55.41. Registered Nurse with Associate's Degree and qualified experience: $34.93 with potential career advancement to $52.43.

Prior to hire, finalist will be required to pass a background investigation and reference check. A standard one-year probationary period applies. Please note: This position is considered a safety-sensitive position due to the essential job duties in accordance with . Examples of Duties
- Assumes the ongoing and systematic collection, analysis, and interpretation of health data in the process of describing and monitoring an individual, family, community and/or systems health event.
- Investigation of disease and other health event.
- Outreach through communication with the community.
- One on one case finding for individuals and families most at risk.
- Perform targeted, mass, or periodic screenings using various tools and trainings.
- Referral and follow-up with individuals, families, groups, organizations, and/or communities.
- Case management of individuals and/or families.
- Perform delegated functions under the direction of health care practitioners as allowed by law.
- Enhance positive health and prevent illness through health teaching.
- Promote and develop an interpersonal relationship with a community, system, family or individual through counseling.
- Consultation with individuals, families, groups, and/or community groups.
- Collaboration with persons or other organizations.
- Build coalitions between community agencies.
- Facilitate community organizing.
- Act as client advocates.
- Design programs to influence and improve health behaviors of the community we serve through social marketing.
- Develop and enforce policies which address health issues such as tobacco compliance checks, animal control, and public health nuisance.
- Respond to disasters and assist communities in recovery to prevent the spread of disease or injury by recognizing deviations from the norm that might indicate an emergency; communicate clearly within the chain of command; assume functional role according to emergency operations plan; participate in drills/exercises.
- Apply professional skills to a range of emergency situations including but not limited to analyzing surveillance data, implementing isolation, quarantine, mass dispensing, or alternate triage sites, selection and use of appropriate personal protective equipment: communicate with partner professionals in other agencies in emergency response, and participate in continuing education to maintain up to date knowledge in the areas relevant to emergency response including emerging infectious diseases, hazardous materials, and diagnostic testing.
- Assume responsibility for meeting continuing education requirements to maintain licensure and to ensure that self-knowledge increases, is current, and relevant to changing health practices.
- Adheres to general employment conditions such as personnel rules, work and safety rules, dress codes, etc.
- May assist with duties and responsibilities as outlined in the County Emergency Operations Plan and Continuity of Operations Plan.
- May work within the Prairie Lakes Community Health Board with programs that involve cross jurisdictional sharing (CJS).
- Other duties as assigned.
Preferred Qualifications:
- Bachelor's Degree in Nursing with current Registered Nurse license.
- Ability to obtain Public Health Nurse Certification.
- Home and community-based experience
Other Requirements:
- CPR certification or willing to obtain upon hire.
- Driver's License valid in the State of Minnesota
