Social Worker (Program Coordinator)

Job TitleAllied Health ProfessionalDutiesThe duties may include but are not limited to:Marketing the model of care and reaching out to community groups and agencies to promote the medical foster care program. They work closely with VHA Public Affairs Office (PAO) to market the program internally and externally.Utilizing National Directives, application forms, and inspection tools to screen and assess Caregiver applicants wishing to become a MFH Caregiver which includes approving and disapproving MFH Caregivers in coordination with the approving official.Providing oversight during monthly unannounced visits to the MFH and addressing concerns of Veterans and caregivers that supports the MFH model of care.Providing training to social workers, caregivers, students, residents, interns, and fellows from other disciplines regarding the advantages and disadvantages of a medical foster home.Receiving and completing consults from interdisciplinary teams (IDT) and other social workers on complex, difficult cases, using advanced practice skills and expertise.Interviewing and conducting psychosocial assessments; interviewing Veterans and family members and/or significant others to establish facts about the Veteran's unique situation; presenting problems and their causes, and the impact of such problems on the veteran's functioning and health.Developing psychosocial treatment plans in collaboration with HBPC/SCI IDT, including goals for psychosocial clinical treatment.Reviewing all data, subjective and objective, and making a clinical assessment, identifying needs and strengths. They use advanced clinical training, insight, and experience to interpret data and to identify viable treatment options, including admission to an MFH.Establishing a continuing relationship with the MFH Veteran and Caregiver and involved family members.Acting as an advocate when it serves the best interest of the Veteran/family and/or Caregiver.Work ScheduleMonday-Friday; 7:30am-4:00pm. Compressed tour of duty negotiable.PayCompetitive salary and regular salary increases.
