Physical Therapist - Travel Contract

Active Washington State PT license required to start the assignment. CPR certification required. Home health experience required: conducting SOC, evaluations, ROC, recertifications, and subsequent visits. Experience with HCHB EMR preferred. Ability to cover patients within a 50 mile radius from base location. Proof of eligibility to work in the U.S. Assignment Details Facility Type: Home Health. Shift: Days (08:00–16:00). Schedule: 5x8, 40 hours per week guaranteed. On-call/Weekend Requirement: No. Mileage reimbursed between patient visits from first to last patient. Primary responsibilities: SOC, evaluations, return-of-care visits, recertifications, and subsequent visits. Dress code: Business casual.
