Transitions of Care RN Case Manager

To enhance patient navigation through the healthcare system, the full-time Transitions of Care RN Case Manager will provide telephonic, short-term care management for high-risk patients post-discharge, advocating for their needs and coordinating necessary resources remotely. Key responsibilities Engage with newly discharged patients to assess their needs and develop personalized care plans Collaborate with healthcare providers to ensure coordinated care and smooth transitions during the post-discharge period Educate patients on chronic conditions and facilitate access to community resources for better health outcomes Required qualifications Unrestricted Compact RN license in the United States, with the ability to secure additional state licenses 3-4 years of RN care management or telephonic case management experience Availability to work Monday-Friday from 8:30 AM to 5:00 PM MST/PST Experience in health insurance, particularly Medicare Advantage Strong analytical thinking skills to address individual patient needs effectively
