We are looking for dynamic, team oriented LPNs who enjoy working with a variety of patients to assist them with obtaining optimum function either within the LTC setting or to assist them with their successful return to home from our active Rehabilitation Unit./ppPurpose: To conduct and coordinate all nursing activities on the assigned unit in a safe, timely and accurate manner in accordance with state and federal requirements and the policies and goals of this facility./ppDuties and Responsibilities:/pulliReceive and give accurate and concise report on each resident/patient at change of shift/liliGive report to C.N.A.s at beginning of shift and receive from them before end of shift/liliOversight and management of C.N.A.s assigned to that unit, including daily assignments, setting and monitoring breaks and evaluating needs for improvement/liliAuthority to discipline/counsel C.N.A.s/liliManagement of all resident/patient care on that unit, including updating nursing care plans and C.N.A. care plans with changes and new orders/liliReview care plans for appropriate problems, approaches, goals and revisions/liliReview C.N.A. documentation, vital signs, bathing, weights, IOs, BMs/liliComplete nursing summaries and quarterly assessments as assigned/liliMake daily rounds and is aware of all resident/patient conditions on that unit and reports pertinent data to Unit Manager or designee/liliNotify MD/NP and resident/patient responsible party of significant changes in condition/liliEvaluate resident/patient needs and the effectiveness of care provided/liliPromote effective restorative nursing techniques/liliMaintain universal precautions and infection control policies/liliDocument completely and accurately using only standard abbreviations/liliAdminister and record medications as ordered and maintain accurate narcotic count according to pharmacy and facility policy/liliMaintain/order/obtain adequate medications, nursing supplies and equipment as necessary for the unit/liliAssist with editing and end-of-month changeover as assigned/liliWeekly skin checks and administration of treatments as ordered, documenting changes and progress/liliFinger stick blood sugars as ordered with accurate documentation and notification of MD/NP if outside parameters/liliObtain specimens, follow up with lab and notify MD/NP of results/liliAssist with meal trays, call lights and alarms/liliClean, re-stock and empty trash on medication and treatment carts before change of shift/liliContact MD/NP/families as necessary/liliTranscribe new orders promptly and accurately/liliAdmit, transfer and discharge residents on assigned unit and notify business office and dietary of changes/liliComplete incident reports and document in the medical record according to facility policy/liliComplete nursing log at end of shift/liliAssist with orientation and staff development/liliParticipate in staff meetings and continuing education opportunities/liliDeliver direct resident/patient care when necessary/liliAssume supervisory responsibilities if requested by the DNS or designee/liliPerform other duties as assigned/li/ulpBenefits:/pulli403B/liliDental insurance/liliDisability insurance/liliHealth insurance/liliLife insurance/liliPaid time off/liliEmployee Discounts/li/ulpSchedule:/pulli8 hour shift/liliDay, Evening, or Night Shift/liliHolidays/liliWeekends/liliFull-time/li/ulpSalary: $29.00 - $45.00 per hour based on years of experience and differential of shift worked./ppSign on Bonus!/ppBonus Structure/pulli$1,000 after 90 days of employment from date of hire/lili$1,000 after 6 months of employment from date of hire/lili$1,500 after 1 year of employment from date of hire/li/ulpRequirements:/pulliNursing: 1 year (Preferred)/liliLTC: 1 year (Preferred)/liliLicensed Practical Nurse (LPN) (Required)/liliComputer literacy is desirable/liliExcellent communication and interpersonal skills/liliMust meet the general health requirements set forth by the policies of this facility/liliAble to push, pull, move and/or lift a minimum of 25 pounds for at least 50 feet with or without the aid of mechanical devices/li/ul/div
