Job Summary: A servant heart caregiver focused on the needs of others. Detail oriented and thorough. Part of an interdisciplinary team making a difference in the community and appreciates work-life balance. Provide direct care to homebound patients in both community and assisted living settings.
Responsibilities and Duties:- Assess patient needs and develop individualized treatment plans based on physician orders and interdisciplinary team input.
- Provide direct speech therapy services to patients in their homes or assisted living facilities within a defined geographic area.
- Ensure timely documentation and compliance with all Paradigm HomeCare policies and procedures.
- Collaborate with physicians, nurses, and the care team to support optimal patient outcomes. Qualifications & Skills:
- Master's Degree in SpeechLanguage Pathology from an accredited university
- Active SpeechLanguage Pathologist license in the State of Texas
- Current CPR certification
- Valid Texas Driver's License and automobile insurance
- Minimum 3 years of experience as a Speech Language Pathologist, with at least 1 year of recent home health experience (within the last three years)
- Knowledge of WellSky EMR software desired Ideal Candidate Will Be:
