MDS Coordinator- RN OR LPN

We are seeking an experienced full-time MDS Coordinator to join our dedicated team at Cedars of Lebanon. As an MDS Coordinator, you will play a crucial role in coordinating the RAI process, ensuring the timeliness and completeness of the MDS, CAA's, and Interdisciplinary Care Plan.
Responsibilities:
- Monitor and guide the completion of assessments in a timely manner
- Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtaining receipt of validation
- Schedule and facilitate care plan conferences and monthly QM meetings
- Manage Care Planning Process
Mandatory Qualifications:
- Must hold a Nursing Degree from an accredited college or university
- Current, unencumbered, active RN license or LPN and CPR
- At least 3 years' experience in Long Term Care as a MDS Coordinator
- RAC-CT Preferred
Desired Skills and Qualities:
- Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement
- Excellent written and verbal communication skills and critical thinking abilities
- Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility
- Ability to organize and prioritize
