About the Role
As a Physical Therapist, you will administer personalized treatment plans in various care settings, including outpatient rehab, skilled nursing facilities, and assisted living communities. Whether you are a seasoned professional or a recent graduate, you will receive the mentorship, guidance, and teamwork needed to advance your career and make a meaningful difference.
Key Responsibilities
- Develop and carry out individualized physical therapy programs as prescribed by physicians
- Document evaluations, progress notes, and discharge summaries accurately
- Work collaboratively with nursing and rehabilitation staff to improve residents functional abilities
- Participate in interdisciplinary care planning meetings
- Keep thorough treatment records, monitor patient data, and procure necessary supplies
- Offer or lead training sessions for staff members
- Provide leadership support in the absence of the Physical Therapy Director
What We're Looking For
- Graduate of an accredited Physical Therapy program
- Active NY State Physical Therapy license
- Passion for working with older adults, subacute patients, and adult populations
- Prior experience is beneficial but not requiredrecent graduates are encouraged to apply!
- Demonstrated leadership skills and ability to supervise support staff when necessary
