Registered Nurse - Transplant Care Coordinator - RN: Post-Heart Peds/Adult

Location & Department Location: Rochester, MN Remote: No Department: Nursing Responsibilities The Transplant Coordinator Nurse is accountable for the coordination of all aspects of care for the transplant patient population. Assesses and plans in collaboration with the patient, caregiver, and multidisciplinary healthcare team to meet needs across the care continuum. Assess patient data, interpret information relative to age-specific needs, and provide patient and family education consistently. Provides leadership through community involvement, committee work, and participation in quality improvement and research. This role is eligible for TN sponsorship; successful sponsorship will require state licensure and completion of VisaScreen or equivalent certification. Qualifications Graduate of a baccalaureate program accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE). Minimum 1 year of clinical RN experience required; 3 years preferred. Must have a current RN license in the applicable state. BLS (Basic Life Support) certification valid for at least 90 days post-start date. Additional Qualifications Work flexible hours and adapt to unpredictable situations within the patient care setting. Demonstrated leadership ability and interpersonal effectiveness. Ability to work autonomously and as part of a team. Strong communication, critical thinking, problem-solving, and computer skills. Certification Requirements Coordinators in Transplant must obtain either Certified Clinical Transplant Coordinator (CCTC) or Certified Clinical Transplant Nurse (CCTN) within three years of hire. Coordinators in Bone Marrow Transplant must obtain Adult or Pediatric Oncology Certification (OCN or CPHON) or Bone Marrow Transplant Certified Nurse (BMTCN) within three years of hire. Recertification must be maintained for the duration of the employee’s tenure. Compensation and Schedule Hourly rate: $39.86 – $58.84 Full-time, 64 hours per pay period. Schedule: Sunday–Saturday, days, 8‑hour shifts, on-call 1–3 times per pay period. Weekend schedule: 1 of 10 weekends.
