Outpatient Physical Therapist

This is a 13-week contract position.

5x8.0 - 8:00 AM to 5:00 PM

The Outpatient Physical Therapist will evaluate all assigned patients and develop, implement, and modify treatment plans in accordance with patients' needs, goals, and physician directives. Responsibilities include delegating and directing all discipline-specific therapy services for those patients and designing functional treatment programs that facilitate effective returns to community activities. The therapist will also serve as an educator for patients, families, students, and coworkers.

Responsibilities include:

• Conduct comprehensive evaluations of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals.
• Complete medical record documentation accurately and timely, including goal writing and functional quality indicator assessments.
• Ensure care is provided under signed, valid physician orders and within insurance approval.
• Participate in a student program, providing supervision and evaluations when appropriate.
• Create an individualized professional development plan and actively pursue professional growth through continuing education and self-study.
• Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
• Understand and participate in organizational performance improvement initiatives.

Required Experience / Certifications / Licensure include:

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
