Licensed Master Social Worker - Hybrid Commack, NY - Full Time or Part Time

PM Pediatric Care is seeking a Licensed Master Social Worker to join our Behavioral Health Program. This opportunity provides comprehensive behavioral health for common conditions seen in primary care such as depression, anxiety, ADD, and conduct disorders within an extended primary care collaborative model. Summary Schedule options: 5 days 1:00pm-9:00pm M-F; or 4 10-hour days, 2 days 11:00am-9:00pm, 2 days 8:00am-6:00pm. Saturday hours are also available. Part‑time positions: 20 hours per week (benefits eligible). This is a hybrid position and requires one or two days in office. Responsibilities Perform psychosocial assessments that comply with evidence‑based practices. Provide counseling and therapy for patients using evidence‑based treatment modalities. Serve as an individual, family, and group therapist. Maintain professional development as required by licensing board. Complete required documentation and paperwork, including biopsychosocial assessments, treatment planning and implementation in a timely manner. Work closely with patients, families, and the care team to overcome obstacles in treatment. Maintain compliance with all company policies and procedures. Demonstrate cultural competency and commitment. Respond quickly to an urgent or emergent need. Maintain a caseload of 30-35 patients weekly and a minimum of 5 patients daily. Qualifications Master’s Degree in Social Work or a related field. Relevant therapist license in the state(s) practicing (LMSW-NY). Experience with pediatric and adolescent patients. Experience providing teletherapy to children and adolescents preferred. Knowledge of medical terminology. Ability to effectively interact, work, and develop meaningful relationships with people of various cultural backgrounds. Knowledge of the psychosocial aspects of illness and health. Strong verbal and written communication skills. Excellent short‑term counseling skills. Experience with discharge planning.
