Field Home Health RN Case Manager - Fort Worth

Tom Trese Networking LLC is seeking an experienced Home Health RN Case Manager to cover Fort Worth, TX and the surrounding Tarrant County area. This field role requires strong assessment skills, documented accuracy, and the ability to manage a patient caseload in the home.
The ideal candidate will have an active Texas RN license, recent home health case management experience, and solid knowledge of Medicare home health, OASIS, and plan-of-care management.
