Afterhours RN Care Manager

SouthernCare, a Gentiva company, is seeking an Afterhours RN Care Manager to provide compassionate hospice care during evenings, nights, weekends, and holidays. The RN coordinates and delivers skilled nursing care, triages urgent calls, performs patient visits, manages symptoms, and implements individualized care plans in collaboration with an interdisciplinary team. This role educates and supports families, ensures timely response to crises, documents accurately in the EMR, and communicates with on-call physicians. The nurse upholds hospice standards, promotes comfort and dignity, and represents SouthernCare’s values in every patient and family interaction.
